First, please rename this application to include your name and the date of your application

Please format the name of the notecard like this....

Joe Smith (your name) AMD MODEL SEARCH APPLICATION month/day/year

I think you'll get the idea. We need to know who you are when you drop the notecard, so you can be contacted the proper way. Now the tricky part, you will need to return the whole renamed notecard to Italia Villota, no later then Midnight on June 6th 2008. All submitted after will be declined or deleted.

IF YOU HAVE A FEW PICTURES OF YOURSELF THAT YOU LIKE PLEASE INCLUDED THEM BY ATTACHING THEM TO THIS NOTECARD, ALSO PLEASE NOTE THAT THE PICTURES MUST BE SNAPSHOTS TAKEN BY YOU, ONE FULL BODY AND ONE HEAD SHOT.
